Community Care Solutions is a private limited company. Our Head Office is in St. Neots, Cambridgeshire.

The company operates thirteen residential homes specialising in the provision of care to adults with Learning Disabilities and associated challenging behaviours. The responsible individual for all the company’s homes is our Managing Director, Mark Constant, who is licensed by the Care Quality Commission (formerly the Commission for Social Care Inspection).

Mark Constant has over 20 years’ experience in commercial management, 14 of which have been within the social care industry. His qualifications include a Masters Degree in Business Administration, NVQ level 5 in Management and a Diploma in Management Studies. Mark is a Member of the Chartered Institute of Marketing and is also a member of Ceretas; the professional association dedicated to individuals working in Home Care.

Management & Staffing

Each of our Homes has a Registered Manager and each is an experienced and highly qualified care professional. All our managers hold, or are working towards, their NVQ IV and Registered Managers Award. All managers are accredited by the Care Quality Commission.

Each home also has an appropriate number of Deputy Managers, Senior Support Workers and Support Workers. All our staff have undergone a rigorous interviewing and checking procedure which includes full referencing, enhanced CRB checks and enhanced POVA checks. All staff receive what is probably the most comprehensive Skills for Care-approved training programme in the care sector. All staff are fully conversant with the company’s philosophy and values. In addition, the company’s Training Co-ordinator ensures that all staff receive at least 5 days’ additional training each year to develop their core skills. We actively encourage and assist our staff to achieve their relevant NVQ qualifications.

Assistance is available to Service Users throughout the night by a member of the care team who sleeps on the premises. Additionally, a senior member of our management team is always on call. 

Service Users at Community Care Solutions

Anyone over the age of 18 and under the age of 65 years, who, because of a Learning Disability, requires help and support with daily living, is entitled to apply to live at one of our homes.  Two of our homes offer special support to younger adults in transition from children’s to adult services.

Referrals

Anyone enquiring about services at a Community Care Solutions Home is provided with an information pack. If necessary this can be produced in different formats or languages. All enquirers are made aware of current Service Users at the home and are encouraged to visit. Most referrals usually follow a Needs Assessment made by the Local Authority, Social Services or the individual Care Manager who will work with the Home Manager to assess the Service User’s individual needs.

Admission Process

When someone is thinking of moving to one of our homes we encourage an initial visit to ensure the home is a place they would like to live. This gives staff the opportunity to meet the individual and the individual and their family or advocate to meet other Service Users and staff. A full explanation of the process is then discussed in detail.

Care Provision

Each Service User has a Person-Centred Care Plan that provides the basis for which care and support services will be delivered. Each person’s plan includes a description of their preferred daily routine, their likes and dislikes and any specific dietary requirements or similar needs. The Care Plan contains risk assessments and any risk management plans and also includes details of social interests and activities and how these will be catered for, together with any arrangements to attend religious services. The plan also includes details regarding contact with relatives, friends and representatives and integration with the general community. Each Service User is allocated a member of staff to act as their key worker. Key workers are responsible for monitoring, reviewing and co-ordinating each Service User’s individual Care Plan. Day Care activities provided to the Service User can include work placements, college, therapeutic care and community activity.
