About Us

Community Care Solutions is a private limited company, whose Head Office is at Brook House, Brook Street, St. Neots, Cambridgeshire.

The company operates a number of residential homes specialising in the provision of care to Adults with Learning Disabilities and Associated Challenging Behaviours. The responsible individual for all the company’s homes is our Managing Director, Mr Mark Constant, who has been registered by the Commission for Social Care Inspection to operate such homes.

Mark Constant has over 20 years experience in commercial management, 14 of which have been within the social care industry. His qualifications include a Masters Degree in Business Administration, NVQ level 5 in management and a Diploma in Management Studies and MCIM (Member Of The Chartered Institute of Marketing). In addition, Mark is a member of C.E.R.E.T.A.S.

Management & Staffing

Each Home has a Registered Manager. Each Manager is an experienced care professional with various qualifications ranging from RMHN to CQSW. All Managers either have or are working towards their NVQ IV and Registered Managers Award. All Registered Managers have been accredited by the Commission for Social Care Inspection, as fit and able individuals to carry out their important role.

In addition to the Registered Manager, the homes employ Deputy Managers, Senior Support Workers and Support Workers as appropriate. There are always members of the care team available throughout the night and usually another member of the team sleeping in and a senior member of our management continually on call. All staff receive a thorough skills for care approved induction training and are fully aware of the company´s philosophy and values. In addition the company´s Training Co-ordinator ensures that all staff receive at least 5 days training per annum to develop their core skills and to assist them to work towards achieving their relevant NVQ qualifications.

Service Users at Community Care Solutions

Anyone over the age of 18 and under the age of 65 years, who, because of a learning disability, requires help and support with daily living are entitled to apply to live at one of our homes. Two of our homes offer special support to younger adults in transition aimedt 16 to 25 year olds.

Referrals

Anyone enquiring about services at a Community Care Solutions Home are provided with an information pack, if necessary this can be produced in different formats or languages. All enquirers are made aware of current Service Users at the home and are encouraged to visit. Most referrals usually follow a needs assessment made by the local authorities, social services or the individual Care Manager who will work with the Home Manager to assess the Service Users individual needs.

Admission Process

When someone is thinking of moving to one of our homes, they usually come for a visit or two to check that this is the place they would like to live. This gives staff the opportunity to meet the individual and the individual and their family or advocate to meet other Service Users and staff.

Care Provision

Each Service User has a person centred Care Plan which provides the basis for which care and support services will be delivered. Each person´s plan includes a description of their preferred daily routine, their likes and dislikes and any specific dietary requirements or similar matters. The Care Plan also contains risk assessments and any risk management plans. Also includes details of social interests and activities and how these will be met and any arrangements to attend religious services of their choice or for contact with relatives, friends and representatives or the general community. Each Service User is allocated a member of the care staff to act as their key worker. Key workers are responsible for monitoring, reviewing and coordinating each Service User individual service care plan. Day Care activities can include work placements, college, or community activity.

Community Care Solutions are committed to providing a quality service and quality care to all our Service Users. We therefore guarantee that:

All staff work as part of a professional team dedicated to providing a quality service in accordance with our Operational Policies and Procedures and CSCI Requirements.
All homes will be maintained and decorated to the highest levels to ensure a safe, comfortable and attractive environment.
All Service Users will be assessed and their care needs formally reviewed at least once per year.
All staff will be trained as appropriate, and be conversant with all of our policies and procedures.
All staff will work to our policies and procedures that have been developed to ensure best practice and to support this charter.
All staff will respect individual Service User´s personal, religious and cultural needs.
All referrals will be treated on equal merit and with equal courtesy.
All enquiries for information will be acknowledged and answered within a maximum of five working days.
